A digital version of this assessment is available as a Google Form. Make your own copy, share it with raters, and collect responses automatically in a Google Sheet.
Make a copy of the form
Google will ask you to confirm. Click "Make a copy" and the form will appear in your own Google Drive. You own it fully and can edit it if you need to.
Set up your session code before you share
The form asks each rater to enter a session code at the start. This is how you group responses from the same organisation together. Before you send the form to anyone, create a simple code for this round — something like your company initials and the month, for example ACME-MAY26. Share that code with everyone you ask to complete the assessment. When you open the results spreadsheet, you can filter by session code and all responses from this round sit together, ready to compare.
Connect a results spreadsheet
Open your copy of the form, click the "Responses" tab, and then click the green Sheets icon. Google creates a spreadsheet that collects every response automatically. Each row is one rater. Each column is one question. The session code is always in the first column, so sorting or filtering by that column groups your raters together.
